360 assessments are used to evaluate an individual, comparing their own answers with those from people that know them. Because in in the business-world these people are hierarchically above, next to and below them, it refers to the 360 degrees of a circle. Which in turn implies "the people around you".
This guide will teach you:
1. Add your questions
First add a question to identify how the respondent relates to the person that's being evaluated. In this example we recommend using a Text Choice question.
Next, add your questions to the questionnaire. For this kind of assessment, you'll get the most out of your report when you're using numerical values. For this guide, we'll focus on numerical sliders, but note you can use a lot of question types if you assign custom scores to the answer options.
For this example we'll score the individual on 3 topics: Knowledge, Motivation and Communication. We'll just ask one question to determine the score to avoid making this guide more complex.
2. Create formulas
Once your questions are ready, head over to the Extra Options and go to the Formulas tab. Create new formulas to calculate the value of each topic you're measuring. You'll also have to do this if a topic is calculated by a single question's result. If you are making a questionnaire where topics is measured by multiple questions, this is where you combine those results.
As our example only uses 1 question per topic, add the response to each of the 3 questions into a separate formula.
Scroll down and configure each of these formulas as follows:
- Enable "Include multiple responses in this formula calculation"
- Enable "Apply filter set when downloading the report"
- Don't set a formula filter on these formulas

3. Set up predefined formula filters
Using predefined formula filters you'll be able to segregate and aggregate your data much faster in the report. Create a filter for each group you'll be evaluating separately. If you want to show the responses of their manager or the average of their peers, this will help you do that.
Create a new filter per group of people you want to segregate or aggregate in the report. Filter on the question you used to identify the responses.
